Role Description

A Virtual Care PTA provides support to the Complex Care Team Program at Concierge Home Care. A Virtual PTA participates and collaborates with the clinical team for oversight of patients identified as high risk for hospitalization.

• Perform virtual visits
• Assess psychosocial needs
• Monitor telehealth
• Facilitate patient needs across the continuum of care
• Provide education concerning relevant medical diagnosis and factors that would increase the risk for hospitalization
• Track data
• Attend required rehospitalization-related meetings, conferences, and symposiums throughout our agency and communities
• Track data, identify hospital admission trends, and provide patient and staff education to prevent hospitalizations
• Assist with innovation and implementation of protocols and guidelines for Concierge Best Practices Programs
• Manage virtual care and telehealth systems by providing virtual assessments, education, and data tracking

Qualifications

• Florida Physical Therapist Assistant License (Required)
• CPR Certification (Preferred)
• Microsoft Office preferred
