Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team on a part-time basis, working every other Saturday and Sunday. The ideal candidate will bring a strong background in various nursing disciplines, including long-term care, assisted living, and acute care settings. This role offers an opportunity to deliver high-quality patient care while supporting our residents' well-being in a dynamic and respectful environment.
Duties
- Provide comprehensive nursing care to residents, including medication administration, vital signs assessment, and physical examinations.
- Assist with patient assessments, documentation, and medical records management using electronic health record (EHR) systems such as Epic or Cerner.
- Support residents with activities of daily living, including tube feeding, catheter care, and basic life support procedures.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to residents’ needs.
- Monitor for signs of acute pain or infection and respond promptly with appropriate interventions.
- Assist with discharge planning, triage assessments, and coordination of hospice or palliative care services when necessary.
- Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information.
- Maintain sterile techniques during procedures such as venipuncture, phlebotomy, or IV insertion while adhering to infection control protocols.
Experience
- Valid LPN license with current state licensure required.
- Prior experience working in long-term care facilities, nursing homes, assisted living communities, or hospital settings preferred.
- Familiarity with EMR/EHR systems like Epic or Athenahealth is advantageous.
- Experience in critical care environments such as ICU or Level I Trauma Centers is a plus.
- Knowledge of ICD coding (ICD-9/10), medication administration protocols, and medical terminology essential for accurate documentation.
- Background in dementia care, Alzheimer's or memory care, hospice services, or acute pain management highly desirable.
- Demonstrated ability to work independently while collaborating effectively within multidisciplinary teams dedicated to senior and special needs populations.
